Creative Insurance Solutions, Inc. respects your right to privacy.  During the writing of an application and subsequent renewals we many request personal and privileged information about you from various sources.  Any information which we have or may obtain about you or other individuals listed as policyholders on your policy will be treated confidentially.

During the course of our business practices, we may disclose information that we collect about you and others listed on your application to insurance companies and medical care institutions for the purpose of claims handling, servicing, underwriting and insurance marketing.

We restrict access to personal and privileged information about you to those employees who need the information to provide products or services to you.  We maintain physical, electronic and procedural safeguards to protect your information. 

You have the right to see the personal information that we have in your file and the right to correct any information that may be wrong.  You must submit a request in writing to Creative Insurance Solutions, Inc., 2951 S. Pacific Avenue, Ste. A   Yuma, AZ 85365.  The request should include your name, address, telephone number and social security number along with the information to which you would like access.

This privacy statement describes our privacy practices for both current and former clients.  We will provide one copy of this notice to joint policyholders, please share this information with everyone covered by your policy.
